Add missing intializations

These were found by cppcheck:
Member variable 'x' is not initialized in the constructor.
The crash #9788 would not have happened if this had been done earlier.

Store ASCII data in std::string
...
Math inset names and LaTeX requirements are always ASCII. This avoids some
conversions and makes math requirements more similar to text requirements.

- add support for \ddddot
- add partial support for inverse diagonal dots (i. e. they are recognized and
displayed correctly, but the needed package is not loaded, because it would
need an off/auto/on setting as amsmath, and that calls for a general solution)

* Inset:
...
- get rid of the cached Dimension. Text inset dimensions are saved in ParagraphMetrics and mathed maintain their own dimension where needed.
- width(), ascent(), descent(): deleted.
- dimension(): now needs a valid BufferView.
- metrics(): now void.
* BufferView::getCoveringInset(): simplify.
* ParagraphMetrics(): now cache inset dimensions.
