Cleaned up the GraphicsCache mechanism, made it use shared_ptr for cleanliness

Cleaned up cruft in InsetGraphics.

Added the auto-conversion of images to InsetGraphics, this adds support
for bitmap images.
Modified the mechanism of image loading for inline viewing - still not
working though.
