5de60e3c77 DocBook: implementation of LATEX_BIB_ENVIRONMENT.
Fixes endless loops when such environments were used.

Add Kornel's test case that triggers an infinite loop when generating as DocBook 5

a6b07608d8 Rename XHTMLStream to XMLStream, move it to another file, and prepare for DocBook adoption.
xml.cpp/h also merges functionalities from sgml.cpp/h.

e340f46476 Fixup bf7f4d71: Let changeRefsIfUnique handle undo
This allows for a much lower memory consumption than a plain
recordUnfoFullBuffer.

Related to bug 6494.

88a0666d6c Do not scan BibTeX files multiple times in a collectBibKeys() procedure.
Scanning is rather slow, so this improves performance in specific
situations (multiple inclusion of larger files in master/child or
chapterbib context)

We have a false positive here, but annotating with coverity[] comments
is not very convenient. Let's try to use good old assertions instead.

e10db6c7dd Annotate destructors to please coverity
Coverity flags this code as not handling exception that may happen in buffer().

My own analysis is that this can never happen because isBufferValid()
does check whether buffer_ is null.

Any insght appreciated. The commit should be expeanded to more cases, actually.

e15a8f3551 Extend LATEXIFY command param handling and add literal param.
File format change.

This allows for the relevant InsetCommand-based dialogs (such as
citation text before/after, Bibitem label, hyperlink name etc.)
to provide both the input of verbatim code or text that is transformed
to proper LaTeX code.

Some dialogs (Nomencl, Href) already had some methods (although they
could not be toggled), which are now centralized and streamlined.

The initial work of this patch has been done by Georg Baum (see
http://www.lyx.org/trac/attachment/ticket/2751/x.diff)

62d36bf04d Correctly load documents moved elsewhere after save.
It is now possible opening documents that where manually moved to a
different location after they were saved and still produce an output.
Indeed, (hopefully) all needed included files are now still found.
When the moved document is saved again, all paths are accordingly updated.
Of course, for this to work, a document has to be saved in Format 490,
at least.

As an example, after converting the user guide to the last format, it can
be copied anywhere and opened without the need of adapting the paths of
included files or moving them to a proper place.

There is one glitch I am aware of. When moving a child document (but not
the master) the path to the master is correctly updated but it won't be
recognized as such. This is because LyX checks that the parent actually
includes this document but, of course, being the parent document not
touched, it appears not including this child. Anyway, it will also occur
when saving the child to a different location and the user is warned
on the terminal about this fact when the moved child is loaded.
However, there is no problem when it is the master that has been moved.

fdbe775b9f This is the result of an audit of all static variables, looking
for possible thread conflicts, of the sort Georg resolved at
6a30211f. I have made static variables const where possible,
and marked cases that looked potentially problematic with the
comment:
	// FIXME THREAD
Many of these definitely are vulnerable to concurrent access, such
as the static variables declared at the start of output_latex.cpp.
Suppose, e.g., we were outputting latex and also displaying the
source of a different document.

I'd appreciate it if others could grep for "FIXME THREAD" and see
if some of these are harmless, or what.

ecef54500d Introduce max_length parameter for plaintext() output routines,
so we can write a limited amount when using this for TOC and
tooltip output.

This should solve the problem with slowness that Kornel noticed,
which was caused by our trying to write an entire plaintext
bibliography every time we updated the TOC. We did that because
he had a bibliography inside a branch, and we use plaintext for
creating the tooltip that goes with the branch list.

Other related bugs were fixed along the way. E.g., it turns out
that, if someone had an InsetInclude inside a branch, then we would
have been writing a *plaintext file* for that inset every time we
updated the TOC. I wonder if some of the other reports of slowness
we have received might be due to this kind of issue?

207d7e4ada Fix bug #8156.
We want the key as id, not the label (which is optional).
We also need a kind of namespace for the citation ids.
We should also clean the id tag before using it.

1a6c599917 Extend the notermination flag to math as well.
Math commands need it as well as text commands. At the same time, this
further unifies the checking for termination and fixes cases of wrong
output (e.g. for 0x2005).

1e947e3a18 Move the citation engine machinery to layouts.
Add a new layout syntax CiteEngine to define the citation commands
available for a given citation engine.

Also extend the CiteFormat syntax to allow more customization. This
mechanism, previously used to produce bibliography entries in the
citation GUI based on the BibTeX entrytype, is now also used to
produce the textual labels for citation insets in the buffer view.

Thus citation styles are almost completely customizable by modules.
Modules for the basic, jurabib and natbib engines are implemented.

Layout format incremented to 37.

a0f6d3db9e Update a bibitem label also when it is emptied.
It is valid for a label to be empty, but up to now the bibliography
information was not updated when a label was emptied.

838af80834 Try to finally fix #6930. All the paths that did not come from context
menus were (intentionally) missing, and it turns out they were needed.

Normally all invocations of INSET_MODIFY should trigger a recordUndo now.
Of course all cases have not been tested, but it should be working.

3fffe51506 Revert r36696: We can't collect BibTeX information during the
updateBuffer() traversal. The reason is simple: InsetCitation needs
access to the BibTeX info to calculate its label. But we won't have it
until we get to the bibliography.
I don't think there is any way around that.

840f2baf91 Simplify some of the bibliography update code. There is really no reason
to be passing the BiblioInfo structure around this way. (That's an old
holdover.) That lets us get rid of the non-const masterBibInfo() again.

66be58b074 Proper fix now for bug #6846. The idea, due to JMarc, is to collect
bibliography information in updateBuffer(), rather than doing a wholly
separate traversal. We still do a separate traversal in two cases, as a
full updateBuffer() call just isn't needed there.
