so we can write a limited amount when using this for TOC and
tooltip output.
This should solve the problem with slowness that Kornel noticed,
which was caused by our trying to write an entire plaintext
bibliography every time we updated the TOC. We did that because
he had a bibliography inside a branch, and we use plaintext for
creating the tooltip that goes with the branch list.
Other related bugs were fixed along the way. E.g., it turns out
that, if someone had an InsetInclude inside a branch, then we would
have been writing a *plaintext file* for that inset every time we
updated the TOC. I wonder if some of the other reports of slowness
we have received might be due to this kind of issue?
calls into the file writing routines and out of doExport(). They were in
both places before: Called once in doExport, and then again in the e.g.
writeLaTeXSource()---and then again, actually, in validate().
It is possible this will reveal some missing updateBuffer() calls
somewhere. But it should somewhat speed up View>Source, since we now do
not do an extra set of such calls in that routine. Rather, we rely upon
the Buffer's having been made up to date first, by the updateBuffer()
call after dispatch.

The need for a LayoutPtr arises from the fact that (a) we do not want to give our clients a Layout &, since we do not want them to be able to change our Layout; but (b) we also need to be able to change which layout is ours. So we cannot store a Layout const &. Or so it seems to the compiler.

- odocfstream: properly fix plaintext output by getting rid of ctor with std::string argument as this can be mixed up with std::ofstream(std::string) ctor where the argument is a file. UTF8 is now the default encoding.

- remove META_INSET from Paragraph.h
- move text_ container to Private. I know this put there for performance reason but I don't see any penalty with this move. I guess this is because Buffer loading doesn't use Paragraph::getChar() anymore. I will move performance critical code to Paragraph::Private when/if we detect them.
- replace Paragraph::value_type with straight char_type: this was more noise than anything.

* Layout.h:
- name_, obsoleted_by_, depends_on_ are now docstrings.
All other changes results from the above string -> docstring changes. In particular, we now try to translate the layout names only if possible (i.e. if they are ASCII only).
