e4a483da32 Enable "silent rules" with automake 1.11. This leads to much nicer compilation 
output. The build should still work with older automake versions.

The po/ directory does not have the nice rules, because I do not want to 
modify Makefile.in.in therein.

5f2429b4e2 This move out the GuiLayoutBox class out of GuiToolbar where it was a mess. The new LayoutBox is less of a mess now but still a mess. 
We need a proper LayoutModel class.

fcc8201d2c Document Dialog: move the listings settings to its own pane. 
This shrinks the dialog to a somewhat reasonable size.

605f6bebf2 Implement a warning dialog with a "Do not show this warning again!" checkbox. 
Since Qt does not provide this, we have to roll our own dialog.
Hidden warnings are stored in sessions.

The dialog is currently only used in the case where an included file
has a different textclass then the master (bug 3218).

8a0d98e67e - stdmenus.inc: Phantom can contain other things as well, not only text 
- LyXAction.cpp: updates and sorting
- GuiPhantom: a dialog to set the different phantom types (stolen from GuiNote)

The Phantom feature should now be complete. Thanks to all who contributed and helped.

f276338227 Preliminar InsetInfo dialog. This was done as an exercise to show Bo (an others) how easy it is to create a new dialog. This dialog needs to be filled in, right now, there is just an OK button.

7bb8a5f183 Re-engineer the toolbar support code: 
- We now rely more upon Qt features,
- the initial toolbar positioning defined in "lib/ui/default.ui" is now only used if there is no session or if session handling is disabled,
- The session saving/restoring is window dependent.
