c75d667e78 Organize header files in src\insets. 
- Functions that inherit from the same baseclass are grouped together with a command;
- Functions that inherit should not have a doxy entry;
- Other functions are also grouped together as much as possible;
- Some function don't need to be implemented if they return the same as the baseclass;
- and more.

996505c2ae This patch introduces an optional argument to Buffer::updateLabels(), so 
as to allow us to call the routine when we are preparing for output and
so to do certain things we might not want to do every time. 

This is an abuse of updateLabels(), in a way, but updateLabels() long
ago became the general recurse-through-the-Buffer routine, and to
implement the sort of thing I want to do here in validate(), say, much
of the code in updateLabels()---in particular, the counter-update
code---would have to be duplicated. So I believe this is the best, and
easiest, way to go.

Actual use of the new argument will follow.

c5738e5af6 Restore XHTML output for InsetLabel and InsetRef. There are a couple 
other fixes in here, too.

There is an issue here which I guess I'll deal with later, namely, that
the name attribute for <a> is unavailable in XHTML 1.1, which is what we
need for MathML support, at least in Firefox. Firefox will deal with it
if it's there, but the document won't validate. I'll figure out what to
do about this later.

85503ec1b9 Get XHTML output for InsetRef working, again in a fairly primitive way. We just 
use the label name as associated text, and put it into square brackets. It'd be
nice to be able to do more, but for that we'd need to associate counters with
the labels, which would be nice for display, too. But we don't have that yet.

1e51e87f68 Inset::addToToc(): change signature. Use DocIterator instead of ParConstIterator. The idea is to have more accurate navigation in the paragraph. But this doesn't work yet.

51a5db7110 General support for InsetCommand context menu. For this to work properly I had to disable all actions triggered by mouse right-clicking. This was bad ui in any case.

017ba3c4c7 Sanitize a bit the way InsetRef and InsetCommand button are drawn. 
* Inset::validate(): renamed to initView()

* InsetCommand:
- get rid of unneeded refresh() and updateButtonLabel_
- setParams(): call initView()

* InsetRef:
- implement initView()
- screenLabel(): transfer code to updateLabels()
- addToToc(): prefix name with BROKEN if the reference is broken.

8e9dc9bd2a Introduce a labels&references cache at buffer level. This cache uses the already existing updateLabels() mechanism and thus speedups labels&references toc generation. As a bonus, duplicates labels and broken references are detected and tagged with "DUPLICATE:" and "BROKEN:" in the outliner (or the navigator).

8e9410b3d0 After a hiatus, I'm returning to the rewrite of InsetCommandParams, the purpose of all of this being to make things more flexible, with the ultimate goal being biblatex support and a kind of InsetCommandFlex that will allow user-definable such things. The next step, really, is to fix up CiteEngine so that we can have different sets of parameters for InsetCitation depending upon what engine is in use. (Something like this also needs doing with InsetInclude.) 
This patch reworks the machinery that holds information about what parameters there are and what their values are. There's enough flexibility here that true keyval support ought to be fairly easy at this point. I'll have a peek at that shortly.

07b6198f4c Move the findInfo() and defaultCommand() routines out of InsetCommand and into its subclasses, so that the subclasses know what parameters they want, etc. Also, introduce an "isCompatibleCommand()" routine, so the subclasses can tell us which commands they are prepared to accept.
