when latex fails with a preview. Fixes bug #7303 (IP fails with hyperref).

I guess this deserves to be sorted out better, because we are doing tricky things by deleted the object from itself.
Problem:
PreviewImage? has a member PreviewLoader?. PreviewImage::Impl::statusChanged() calls PreviewLoader::remove. PreviewLoader::Impl::remove removes a snippet from the cache. In the cache is a map of the snippet and a shared pointer to PreviewImage?. This means that removing the snippet from the cache, destroys the PreviewImage?. When we get back to PreviewImage::Impl::statusChanged() this will start to crash.

Buffer whether we are exporting or not, rather than rely upon isClone(),
which could be adapted to other purposes. And, of course, someone might
choose locally to disable cloning....

allow this as a fallback. E.g., if we're unable to export as MathML,
then we try to export as an image.
There are several ways, I am sure, in which this implementation is not
ideal.

- implement an UI to specify the size of InstantPreview images
- change the default size to 1.0 because we've got many complaints that the former default size of 0.9 was too low

- switch to QImage backend instead of QPixmap. In any case this was done internally by Qt for any image loading or transformation. This should relieve the X11 server a bit for big images.
- try to clear out the memory after a transformation by calling QImage::detach(). Unfortunately there seems to be a bug somewhere in Qt... see (http://bugzilla.lyx.org/show_bug.cgi?id=5002).

- odocfstream: properly fix plaintext output by getting rid of ctor with std::string argument as this can be mixed up with std::ofstream(std::string) ctor where the argument is a file. UTF8 is now the default encoding.

Known problems:
- No space is output after a \hfill. I probably broke this with the
InsetCommand patch. I'll have a look later.
- Although the encoding is now UTF8 the arguments of the inputenc package
are still the old ones, so LaTeX will not run.
- Labels and references with non-ASCII characters are broken. This needs to
be fixed in lyx::support::escape(), but this is a file format change.
- Something seems to be wrong with index entries, but this is probably also
due to the InsetCommand changes.

Only qt4 is guaranted to compile and work. I did not remove gtk and qt3 lyx_gui.C because they might be needed for reference to complete the header declarations in "GuiApplication.C".
- lyx_gui::use_gui transfered to lyx::use_gui in lyx_main.C
- all remaining lyx_gui functions transfered to Application and corresponding GuiApplication implementations.

(Converter::Impl::Impl): Don't call the default converter directly,
but create a temporary script with build_script() as for the
configured converters. This makes sure that the file name does not
need to be passed on the command line anymore.
(build_script): This cannot fail anymore, so change the return type
to void
(build_script): Use build_conversion_command also for the default
converter. This has the advantage that the special code for moving
${outfile}.0, ${outfile}.1 is actually used for ImageMagick's convert.
(build_conversion_command): factored out from build_script
