Synchronizing insets asserts with two views open

This finishes the change of http://www.lyx.org/trac/changeset/28587.

Replace the setButtonLabels functions that need a BufferView by buttonLabel functions. These functions modify the label at drawing time.

Crash with completion in mathed.

When deleting an inset, the stored DocIterator might contain a reference to a non-existent Inset and Text. Therefore, if we detect that the depth has changed, we are (for sure) not in the same paragraph, but we should make sure not to ask for the innerParagraph.

The work-around to clear the clipboard was needed to work around bug 4846. This bug has been resolved by the fix for bug 5472, so the work-around isn't needed anymore.

GetStatus of an InsetCollapsable asserts for unhandled commands.

When you add a command to the context menu of an InsetCollapsable of which the getStatus method does not handle this command, an assertion will be raised. This happens when getStatus doesn't handle a command it is forwarded to text::getStatus which asserts because cur.text() is the text the inset is in, while this is the text of the inset. So, we only dispatch to text_ if the cursor is inside the text_. This is not the case for e.g. context menus.

Synchronizing insets asserts with two views open

It changes the autoOpen_ member of InsetCollapsable into a map, such that an autoOpen value can be specified for each bufferView. Now, the assertion is avoided and insetCollapsable can be open in one bufferview and be closed in the other in very special cases that the cursor end up in a closed inset.

Compare with the MathMacro::editing_ member.

90feb482db Fix bug 5787-part_2 (Labels in Listings Children Don't Update Properly). 
It was still possible to set the label of an included Program Listing to an already existing label (if there was no label specified yet).

0aca981637 Fix bug 5699 (crash when using outliner in child document while the master is not opened). 
The problem was that, if the master is not opened, no respective guiDelegate exists, and the functions that update the toc do not trigger. The fix is to update the child's toc backend and toc items directly in such cases.

Comments:
* -1 is a valid return of bufferCO->currentIndex() so we need to test for it.
* set the correct enabling of the button in case no call to updateContents follows.
* remove unneeded call to setFocusProxy. The widget is either disabled or the focus proxy is set in updateRefs.

52d3a1e426 Move some code around in an effort to make information about modules 
available to tex2lyx. The point is to move the crucial routines that 
deal with modules out of BufferParams.cpp, which tex2lyx does not have,
and into TextClass.cpp, which it does.

This is all cleanup that makes a lot of sense independently.

Note that LayoutModuleList will shortly become a real class.

b9df3d88c9 InsetTabular.cpp: partly reverting r28437, because I was wrong: it is totally valid to have several rows to be the header and also several rows can have a caption, my own EmebeddedObjects manual has examples for this - stupid me! 
Bug 5752 is nevertheless still fixed.

0c3109e2d3 * TocWidget.cpp: 
- improve performance with Outliner open with Qt >= 4.3 by using native methods instead of home-brewn code.

438aaf2ec9 Add a new member to FontInfo denoting the color to be used when painting. 
* rowpainter.cpp: now the color for painting selected text and changed text is set via FontInfo::setColor. However, this color should remain a ColorCode as this color is used in e.g. the GuiCharacter dialog and for LateX output. Therefore I use now the FontInfo::setPaintColor function.

* FontInfo.cpp/h: new members paint_color_ and setPaintColor(). The function realColor is updated to return the paintColor when it is set.

2e7698c4e6 Add a new Color class. This class makes it possible to specify a color as a merging of two other colors. The actual computation of the new color is done in ColorCache::get.

1f58d53deb Bug 5562: Fix command line output of subfigure insets by introducing new OutputParams and LaTeXFeatures members that tell the insets whether they are in a (sub)float. 
Later, this can be replaces by a backpointer approach, as proposed at bugzilla (note that there are already similar cases such as InComment or InTableCell, so a backpointer approach would need to adapt the OutputParams methos anyway).

ac4e461898 Move a couple routines from InsetFlex to InsetCollapsable so the CustomPars 
and ForcePlain tags will work not just with InsetFlex but all InsetCollapsables.

cf40884454 UI Command Buffer improvements. 
* the history is stored in the session file and restored in the next LyX session,
* the lists now disappear when the mouse is released in stead of pressed,
* correct enabling/disabling of the up/down button,
* hide the lists when clicked outside the list. Now using the function event() and check for a mouseButtonPressEvent instead of using mouseReleaseEvent(). The latter is only called when the mouse button is released inside the widget, but we want to use this event to detect a mouseclick outside the list.

7dcc99fb17 Revert r27975 and 28105, as they are causing too many problems, e.g., 
that search gets messed up, and that files can take forever to load.

650a0261a6 Disable the Edit->Math menu when not in math.

Note that getStatus() always returned true for LFUN_BOOKMARK_CLEAR. This is because bookmarks().size() is hardcoded in the code to be 9 which is always larger than zero, obviously

cc3b9c61cf Fix crash when no new document can be created. 
This occurs frequently when the temporary directory cannot be created because it already exists. The temporary directory can exist as a result of previous crashes of LyX.

9d48ad4f74 Improvements to the parser that extracts the "family name". We now get

852c5178de FindAndReplace by Tommaso: 
- solves some issues related to the dialog set-up
 (overlapping of dialog title with upper sub-widget),
 by "borrowing" the widget structure from
 ViewSource.{h,cpp}
- reduces the default occupation of the dialog (in particular,
 it was "colliding" with the ViewSource itself);
- proposes to introduce a tabbed-panel in the F&R
 dialog, so as to have a "basic" and "advanced" tabs;
 (one option could be to make the dialog even smaller
  by moving all of the replace-related stuff into a separate
  "Replace" tab)

- recovers the eventFilter functionality back, so as to catch
 "Enter" or "Esc" key presses while in the Search Work Area

ac1fd9b37f fix bug 2558: hspace support in math 
The basic idea was to reuse the corresponding text space inset in the same
fashion as is already done for references. The dialog displays a different
selection for math than for text. If wanted, the additional spaces could
also be enabled for text, but that would be a file format change.
Constructs like \hspace{\mylengthvariable} that are not supported are
treated by the math parser as ERT as before.

For reasons I don't know the context menu does not work, but this is not so
important IMHO (since a left click opens the dialog).

fa6d2fd93d Currently, a second InsetLayout declaration of a given inset completely 
overwrites the existing definition, instead of updating it, as our Style
declarations do. This patch fixes this behavior, so that e.g.:

After executing DEPM to remove a double space, the position of the cursor was only 'fixed' if the cursor remained in the same par. This patch corrects this
to fix the position of the cursor also when the cursor is in a nested inset which is in the same par. This is needed because otherwise the cursor would not
point to the inset but one pos after it.

39bc3f3f93 Fix most of bug 5539. 
If a file fails to load, we do not try to load it again. The downside is that, even if you fixed the problem, then LyX won't try to load it again in that session. I tried resetting the failedtoload_ variable in fileChanged(), but that didn't work, as it doesn't seem to get hit. I'm not sure what to do here, but it isn't a huge issue.

6b64fc8ca6 * src/Language{.cpp,h}: 
- new member internalFontEncoding() that indicates if a language
	  switches the font encoding internally.

* src/Paragraph.cpp (latexSpecialChar):
	- don't call latexSpecialT1 if the internal font encoding isn't T1.

This fixes the output of straight quotation marks in Hebrew and Greek.

0a2b62f468 According to http://en.wikipedia.org/wiki/ISO-2022-JP, a character may be 
encoded using 1 or 2 bytes, but it may be prefixed by 3-byte escape sequence.
So, a single ucs4 char could need a maximum of 5 bytes. I think that it
is better to be safe than sorry...

552da336ac Now tex2lyx is able to parse stuff that is in unicodesymbols file, as well 
as accents. Time will tell how robust this is.

All support for InsetLatexAccent has been removed.

I did not do the support for \inputencoding yet, because it is more difficult

28f43b1896 Now tex2lyx is able to set the encoding from what it reads in the preamble. 
What works:
- parsing of inputenc should work
- \inputencoding is acted on in the preamble

What does not work:
- \inputencoding in the text
- all the corner cases I have not considered, and all buggy stuff in the
  'what works' paragraph
- InsetLatexAccent are still created, but I do not know when they got added
to the code.

The only notable trick in the code is that I had to disable buffering. Otherwise
the whole text was read before I had a chance to change the encoding...

Finally I remove the artificial limitation that forbid
  \usepackage[opt1,opt2]{package1,package2}

337c6d1577 First serious step for utf8 file format format. tex2lyx is now able to 
read utf8 tex documents and translate them to lyxformat 249.

There is still no code to discover the encoding and use it, but it is the
easiest part (I hope).

bb04efd5a8 Now the document is not parsed at the beginning anymore. 
Instead, new token are read when requested. The idea from now is
to be able to chenge encoding on the fly.

Now it is time to see whether idocstream actually works...

362d6e468f This patch fixes a set of bugs reported by Philippe relating to excluded 
and default modules. The lesson, in the end, is that we need to do all the
removal of modules that no longer `fit', for whatever reason, before we
do anything else. So the patch basically just moves the `removal' code
into a new routine that gets called before the routines that add default
modules and then check consistency.

13cf13c690 fix a bug in visual cursor movement 
The setting of boundary need to take the paragraph's direction (LTR/RTL) into
account.

This bug was discovered by Vincent, while trying to solve bug #5061.

While this fixes the original manifestation of bug #5061, it doesn't really
solve the underlying issue, which is that on a boundary, after typing
Ctrl-Enter, the cursor is painted on the same line as the newline symbol
rather than on the new line. See bugzilla for further discussion of this.

The problem is that, for an unknown reason, an InsetMathHull is used *inside* an InsetMathHull. This is the reason why we have two levels of instant preview. I don't know if that was intended.

When moving the cursor, only the doDispatch() of the enclosing InsetMathHull is
called, not the inner one; so we don't have a chance to detect that the inner InsetMathHull should leave its preview mode.

The new leavePreview() method ensures that the screen will be redrawn if needed with cursor movement.

This is solved by the attached patch.

We should not restore the last used buffer if the active buffer has
changed. Especially not when you cannot choose the buffer which is now
still the case.

72721655c5 Remove some indirection leftover from Andre's removal of controllers and such. 
When we had GuiDocument and GuiDocumentDialog, there was a separation that required
this kind of thing, but now apply() is only ever called from applyView(), which is
private and only ever calls it with our local params.

This simplifies the forthcoming ProvidesModule, etc, patch a bit.

2351615f0a Fix LFUN_LAYOUT_MODULE_ADD: We need to make sure that the module 
isn't already added, that its requirements are satisfied, and that
it doesn't conflict with any loaded modules. Mostly, this just
moves existing code into a new routine where we can get at it.
