The problem was that, if we killed export when some graphic was
being converted, or some external template was being handled, it
would only cancel that process, and then it would just continue.
To deal with that, we need to do a few things:
1. Modify the return values for some of the Converters routines,
and similarly the routines for external templates, so we can
tell when something has been canceled.
2. Throw an exception from InsetGraphics or InsetExternal when this
has happened during export, but ONLY when the Buffer is a clone.
We shouldn't be able to 'cancel' export when we're, say, generating
code for the preview pane, but this keeps us on the safe side..
The exception then has to be caught, obviously, back in the export
routines in Buffer.
Probably Coverity will be unhappy about something here, but I'll
deal with that problem as it arises.
Actually photos (i.e. .jpg files) where supported previously, but for pdflatex
output an unneeded conversion to png was done. The RasterImage templates
behaves now exactly as InsetGraphics for these files: If the input format is
jpg, use that for pdflatex, else convert to png.
This resolves a dependency of src/support/docstream.cpp on src/TexRow.cpp,
which is the wrong way round. This fixes the linking
src/tests/check_ExternalTransforms with MSVC where the linker is not clever
enough to detect that the whole otexstream class is unused.
This was not necessary when LyX was generously outputting newlines.
As it may happen that the output produced by writeExternal (the
result of an external inset) starts right at the beginning of a
line, if otexstream does not know that something is already on
the line, the iomanip-like variable 'breakln' fails to actually
break the line.

(updateExternal): Add new dryrun flag and only record updated files
for the exporter if that flag is false
(writeExternal): Call updateExternal also if dryrun is true and the
product contains the $$Contents placeholder. In this case we need to
run the conversion(s) even in dryrun mode, because we would not
output the right stuff to the .tex file otherwise.

Known problems:
- No space is output after a \hfill. I probably broke this with the
InsetCommand patch. I'll have a look later.
- Although the encoding is now UTF8 the arguments of the inputenc package
are still the old ones, so LaTeX will not run.
- Labels and references with non-ASCII characters are broken. This needs to
be fixed in lyx::support::escape(), but this is a file format change.
- Something seems to be wrong with index entries, but this is probably also
due to the InsetCommand changes.
