6fa0532ed4 Escape special chars in indexes 
We now have the functionality via sub-insets, and unexperienced users
are surprised when they don't get a literal !.

The unescaped chars can still be used via ERT.

76280b84db Fix URL documentation 
It's not true that long URLs are not broken by default.
They are just broken only at certain characters, and this can be customized.

It's also not true that the font formatting cannot be customized.
