e28c88767b OK, now here's a big one: XHTML output for InsetBibtex. It's not perfect--- 
see the notes---but it does work reasonably well. And it will work a lot
better when an unrelated patch of mine goes in: one that generally improves
the display of BibTeX-derived information.

Note how we use the TOC here, which has already gathered the information we
need. This can also be done for other things.

21121f3fb5 It is convenient for me to change this for use in XHTML output, but I also 
have been wanting to change it for a while. It is not very useful to have
a bunch of years listed in the citations TOC (which you get if you have used
\citeyear very often. Better, it seems to me, to see the citation keys.

Of course, this can be switched again if need be, and I can figure out a 
different way to handle my own problem.

ccf91c3bf0 Add a couple more HTML* tags to InsetLayout, in a way that improves the 
footnote output and allows a lot more flexibility. (In some ways, this now
seems to be more like what docbook does.)

658457b847 Rationalize the handling of InsetLayout's preamble and requries. That is, 
bring it into line with the handing of Layout's preamble and requires. This

864773d7d9 Allow literate documents other than noweb to work out of the box. Currently 
only sweave is supported (for building documentation, not programs).

Layout files allow a new OutputFormat tag 

Modules can be conditionned on a feature of the type from->to, indicating that
it should be possible to convert from format 'from' to format 'to'.

Layout format incremented to 15.

852dfe3869 Fix writing of \bibtex_alternatives and \index_alternatives to the 
preferences file. Now the alternatives are only written if they have
actually been changed, and not always as it was before. Existing
preferences files will be automatically trimmed on next save.

70351330a0 Another tweak to account for when both paths end with '/'. 
The problem here is making sure that path_prefix_is() behaves exactly
as would a case insensitive prefixIs(). Anyway, this is still better
than trying to fix the semantics of common_path().

0cb87c6bc4 When the clipboard contains a tabular selection, dirty_tabular_stack_ is true. However, it is set to false when the user clears a selection (without copying). After this (mostly accidental) action, pasting the tabular contents goes wrong. 
So, we only clear the dirty_tabular_stack_ flag when a 'real' copy to clipboard has been made.

22e8bd3c3e Partial fix for bug #5465. This will enable scrolling down past a very large inset. With this code, LyX will scroll up everytime you try to scroll past the inset, because the cursor is set on the uppermost row.

70696182dd Take into account output redirection when spawning a program. 
Now things should behave as before the QProcess era (I hope).
The parser only accounts for output redirection, no fancy
things such as piping are possible, yet.

0ed38ed1f4 Allow to separately redirect stdout and stderr.

a52cac0476 Fix part 2 of bug #5758: Crash when closing the dosbox from which LyX was started. 
A problem occurs when we write the ConverterCache index file. This action is instantiated in the destructor of the 'static ConverterCache' object. The use of QFileInfo at that point leads to a crash.

It is maybe not a good idea at all to put part of the functionality in a destructor, especially not when we do not delete the object ourselves and when this functionality uses external libraries etc.

4b8376bbb3 Revert r29532 and do something less risky. 
Now, the tabs are closed in reverse order, which is aesthetically better. Consequently, we have to open the files (read from the session file) also in reverse order.

6e7031dc5a File names must be compared taking into account filesystem case sensitivity, 
otherwise the same file could be opened in different buffers by simply
changing the case of a character in the name.
