7a6ad48495 Use font metrics for computing the size of math decorations
It was an error using an absolute size instead of one relative to the
size of the used font. It was requiring using the physical dpi of the
screen and caused #12222. Now everything is computed with respect to
the metrics of the font and should be automatically correct.

Fixes #12222.

d999d8e8d9 Improve alignment of math decorations
This patch takes into account the skewness of a character for properly
align a decoration. Unfortunately, the required amount of shift depends
on single chars, so that the recently introduced FontMetrics::italicSlope()
method cannot be used with all characters. A heuristics that produces
satisfactory results is using the italic slope only with characters whose
italic correction (kerning) is zero.

99fb1c7003 Overhaul (no)limits support
- Add limits support to InsetMathScript
- Add limits support to InsetMathClass
- Fix bug where limits changing is disabled in inline math

Now the MathData objects remember whether they are in diaply mode.

Fixes bug #12045.

f96b99dcb3 Implement properly \limits and \nolimits
These are now properties of insets that can be operators :
InsetMathSymbols, InsetMathDecoration (for over/underbrace) and
InsetMathMacro (for its contents).

Each of these has a limit_ member that allows to remember a limit
forcing and a member defaultLimits() that indicates what to do in the
absence of such forcing. Moreover the write() method calls
writeLimits().

This allows to simplify the definitions of integrals in lib/symbols by
defining the integrals as macros of their "op" version, as it is done in
the style files.

Also, many hardcoded assumptions can now be removed.

The handling of LFUN_MATH_LIMITS is now done in InsetNest, which tries
successively to apply the limit change to (1) the character after
cursor, (2) the character before cursor and (3) the character at the
end of the inset (useful for script insets?)

The new code allows to define
  \newcommand\int{\intop\limits}
but not
  \newcommand\makelimits#1{#1\limits}

It is also possible to type explicitly \limits or \nolimits to modify
a symbol.

e30f3d76d2 Bulk cleanup/fix incorrect annotation at the end of namespaces.
This commit does a bulk fix of incorrect annotations (comments) at the
end of namespaces.

The commit was generated by initially running clang-format, and then
from the diff of the result extracting the hunks corresponding to
fixes of namespace comments. The changes being applied and all the
results have been manually reviewed. The source code successfully
builds on macOS.

Further details on the steps below, in case they're of interest to
someone else in the future.

1. Checkout a fresh and up to date version of src/

    git pull && git checkout -- src && git status src

2. Ensure there's a suitable .clang-format in place, i.e. with options
   to fix the comment at the end of namespaces, including:

    FixNamespaceComments:                           true
    SpacesBeforeTrailingComments:                   1

and that clang-format is >= 5.0.0, by doing e.g.:

    clang-format -dump-config | grep Comments:
    clang-format --version

3. Apply clang-format to the source:

    clang-format -i $(find src -name "*.cpp" -or -name "*.h")

4. Create and filter out hunks related to fixing the namespace

    git diff -U0 src > tmp.patch
    grepdiff '^} // namespace' --output-matching=hunk tmp.patch  > fix_namespace.patch

5. Filter out hunks corresponding to simple fixes into to a separate patch:

    pcregrep -M -e '^diff[^\n]+\nindex[^\n]+\n--- [^\n]+\n\+\+\+ [^\n]+\n'  \
        -e '^@@ -[0-9]+ \+[0-9]+ @@[^\n]*\n-\}[^\n]*\n\+\}[^\n]*\n'         \
        fix_namespace.patch > fix_namespace_simple.patch

6. Manually review the simple patch and then apply it, after first
   restoring the source.

    git checkout -- src
    patch -p1 < fix_namespace_simple.path

7. Manually review the (simple) changes and then stage the changes

    git diff src
    git add src

8. Again apply clang-format and filter out hunks related to any
   remaining fixes to the namespace, this time filter with more
   context. There will be fewer hunks as all the simple cases have
   already been handled:

    clang-format -i $(find src -name "*.cpp" -or -name "*.h")
    git diff src > tmp.patch
    grepdiff '^} // namespace' --output-matching=hunk tmp.patch  > fix_namespace2.patch

9. Manually review/edit the resulting patch file to remove hunks for files
   which need to be dealt with manually, noting the file names and
   line numbers. Then restore files to as before applying clang-format
   and apply the patch:

    git checkout src
    patch -p1 < fix_namespace2.patch

10. Manually fix the files noted in the previous step. Stage files,
    review changes and commit.

89662a6852 Re-implement math markers logic.
The goal of this patch is to be able to properly remove the space
needed for markers in the case of insets that are inside macros and do
not need these markers. This was attempted at 9a9a6a8, but did not
work reliably.

To this end, the following simplifications are made:

* instead of drawing its own markers, each inset has a virtual method
  marker() which prescribes either NO_MARKER, MARKER (normal bottom
  marker) or MARKER2 (top and bottom marker). All explicit calls to
  (draw|metrics)Markers(|2) are removed.

* the space necessary for the markers is now counted in the
  before/above margins in the row structure. Therefore painting will
  not happen at (x + 1, y), but just (x,y).

* the methods drawDecoration are removed.

* the helper methods InsetMath::(draw|metrics)Markers(|2) are removed
  and replaced by a new function drawMarkers in MathRow.cpp.

Now the marker type is kept in the MathRow::Element object (and set to
NO_MARKER in not editable context) and the marker is accounted for in
MathRow::(metrics|draw).

Moreover, the extra pixel for the marker is taken on the before/After
space if possible. The marker will only require extra space when
before/after is 0.

See comment 168 of #8883 to understand what issues are fixed.

e35fda62a7 Some code factoring for changing mode.
About removing the changeEnsureMath in MathMacro::{metrics,draw} : as it is only
called in edition mode, it only happened for user macros for which no ensureMath
is needed anyway. ChangeEnsureMath should indeed be applied for global macros,
but since they are linearized there is no obvious place to call it.

02e82157ec Get rid of Inset::setPosCache
This helper method is used to set the inset position cache at drawing
time. However this is already done by the drawing code itself
(MathRow::draw, RowPainter::paintInset), so that there is no need to
do this same work again in a different place.

Note that the inset positions are still set in InsetTabular::draw for
each cell.

This commit comes with a mild risk attached (it might be that some of
these calls were useful after all!), but all of this is fixable.

9a9a6a8c8f Skip drawing of markers in non-editable math data
There is no reason to reserve pixel space in macros replacement text,
which is not editable. This makes macros more compact and eases the
writing of lib/symbols.

* introduce new InsetMath::drawMarkers and friends that do nothing
  when nested inside a macro. This required to move macro_nesting
  inside MetricsBase, and to pass MetricsInfo & to metricsMarkers.

* keep track of nesting when drawing rows or macros.

b2b8733096 Remove a conversion to_utf8() inside FontSetChanger
This requires to change many docstrings into std::strings. The logic behind that
is that they represent a fixed set of math fonts, and therefore “string” means
here “poor man's enum” rather than text (this is consistent with MetricsBase).

Profiling of scrolling inside a document over macro-instensive areas:

Before the patch:
  44,1% BufferView::updateMetrics()
   -> 34,8% InsetMathHull::metrics()
     -> 9,8% FontSetChanger::FontSetChanger()
  28,4% BufferView::draw()

After the patch:
  35,3% BufferView::updateMetrics()
   -> 27,2% InsetMathHull::metrics
     -> 0,4% FontSetChanger::FontSetChanger()
  47,5% BufferView::draw()

FontSetChanger::FontSetChanger() is made 41x less expensive (with reference
BV::draw()) just by removing this conversion. The remaining 0,4% could be
squished by replacing the strings with a proper enum, but this is premature. Of
course, this only treats the symptoms: there is no good reason that this
function is called 45500 times over the time of 40 repaints.

e2bc7ffae3 RefChanger
RefChanger temporarily assigns a value to a non-const reference of any
kind. RefChanger provides a flexible and uniform generalisation of the various
scope guards previously derived from the old Changer class in MetricsInfo.h.

As before, a temporary assignment lasts as long as the Changer object lives. But
the new Changer is movable. In particular, contorsions are no longer needed to
change a private field. Special code can be moved into the appropriate classes,
and it is no longer necessary to create a new class for each specific use.

Syntax change:

   FontSetChanger dummy(mi.base, value);
-> Changer dummy = mi.base.changeFontSet(value);

New function for generating arbitrary Changers:

  Changer dummy = make_change(ref, val, condition);

Bugfix:

* Fix the display of \displaystyle{\substack{\frac{xyz}{}}} (missing style
  change).

699a6db9fa Add some missing member variable initializations
These were all found by cppcheck. Even in constructors that are there "only
because of std containers" the class should be initialized correctly. You can
never know whether such an object does not get used, and then a nice crash
caused by dereferencing a NULL-pointer is better than undefined behaviour.

0a93c9b70f Store ASCII data in std::string

c88eaf877e Changing only the shape does not always work. For example,
\mathbb{\underbar{a}} is not correctly rendered on screen.
We really have to change the font set, but not always.
This cannot be done using an "if" statement, as when
the FontSetChanger scope ends, everything is restored,
defeating our change. Thus, this has to be done in the
FontSetChanger class itself. This is easily accomplished
by introducing a boolean with a default value of true
for really changing a font set.
This will also be useful in other cases that I am discovering.

a0043da05f Introduce and use a "math_flavor" flag with OutputParams.
I'm not happy about having what is basically the same enum in
BufferParams.h and OutputParams.h, but I don't see how to unify them
without either (a) including BufferParams.h in OutputParams.h, which is
ugly or (b) creating some new file just containing this enum, which
seems like overkill. Any other ideas?

9c662bf442 - fix requirements of \dddot
- add support for \ddddot
- add partial support for inverse diagonal dots (i. e. they are recognized and
  displayed correctly, but the needed package is not loaded, because it would
  need an off/auto/on setting as amsmath, and that calls for a general solution)
