5908bb3ea6 Cleanup button handling in InsetInclude
The include inset has its own button_ object, that hides the button_
object of the InsetCommand parent. Remove that.

Remove the hand-made caching that uses the set_label_ boolean.

Let metrics() and draw() use the inherited methods. This requires to
use setBroken as needed and to implement inheritFont.

The use of InsetCommand::draw is what restores the hover effect.

Fixes bug #10415.

ba738d0167 Use new rowFlags() values to remove some inset hardcoding.
The enum DisplayType is replaced with the flags RowFlags that can be
combined. Here is the correspondence between the old DisplayType and
the new Inset::RowFlags:

DisplayType   RowFLags             Meaning
 Inline        Inline               plain inline inset
  --           BreakBefore          row ends before this inset
  --           BreakAfter           the row ends after this inset
 AlignCenter   Display	            the inset is centered on its own row
 AlignLeft     Display | AlignLeft  the inset is left-aligned on its row
 AlignRight    Display | AlignRight the inset is right-aligned on its row
  --           RowAfter             an extra row is needed after this inset

Display is just a shortcut for BreakBefore | BreakAfter.

The flags for the newline inset will be BreakAfter | RowAfter,
while the separator inset will just use BreakAfter.

This groundwork does not introduce any new feature at this point. It
aims to remve the numerous isNewLine and isSeparator all over the
code, and to eventually optional break after some insets like spaces
(see #11621).

Most display() methods are renamed to rowFlags(). Some are removed
because they returned Inline.

Now display() is only a helper function for hull insets.

e4da314522 Try to fix bug #6505.
Keep track of nested includes and just refuse to re-enter a file
we're already in the process of handling.

There's a question whether we should do this in updateBuffer and
validate, or whether we should do it separately. For now, this seems
to work.

f479c5e8c7 cprotect include inset
We \cprotect them in fragile context. This allows \input, \include as
well as \verbatiminput and \lstinputlisting in sections etc.

They still don't work in titling (#2528)

8b9d1b8601 Attempt to fix bug 9158 using updateBuffer.
Along the lines suggested by JMarc, we now collect the list of bibfiles
in use in the updateBuffer routines. This actually does simplify the code
quite a bit. See the discussion there for reasons to go this way.

88a0666d6c Do not scan BibTeX files multiple times in a collectBibKeys() procedure.
Scanning is rather slow, so this improves performance in specific
situations (multiple inclusion of larger files in master/child or
chapterbib context)

cea2d71e64 Comply with rule-of-three
The rule-of-three says that if any of virtual destructor, copy constructor
or assignment operator needs to be manually implemented, then all three
should be implemented. Otherwise you can get subtle bugs which can be
difficult to find. In the changed classes, changing a copy-construction to
an assignment would have had surprising effects. Now they all behave
consistently.

d5a5fbb8ee Enhancements and bugfixes to the TOCs
* New TOC "math-macro". This means that math macros can now be accessed in the
  outline pane in their order of appearance or in alphabetical order, and can be
  searched using the filter.

* Lists of floats now show subfloats deeper in the navigation menu

* The arbitrary 30 element cut-off after which nothing is shown except "Open
  Navigator..." is removed. Menus now have no limit in size, so Qt may display
  them scrollable. In exchange, we always show "Open outliner..." at the
  beginning. I tested for performance issues with a rather complex document and
  it is fine; but this does not exclude corner cases with lots of TOC entries of
  a certain kind. If necessary, populating the navigation sub-menu should be
  delayed like the main menu.

* Elements that do not contribute to the output (e.g. in a note, a disabled
  branch) are now preceded with a symbol indicating this status. (The machinery
  was already there; I wonder why it was not implemented already.) I have chosen
  U+274E NEGATIVE SQUARED CROSS MARK.

* Fix the contextual menus in the outliner (bug introduced at 94e992c5).

* Toc item now move to the caption when present, but first center on the float,
  to prevent the situation where the caption is at the top of the screen and the
  contents of the float is off-screen above the caption.
  (Internally, the action of the toc items can now be customised)

* Fix the LyXHTML output. Disabled captions no longer appear in the list of
  figures.

ecef54500d Introduce max_length parameter for plaintext() output routines,
so we can write a limited amount when using this for TOC and
tooltip output.

This should solve the problem with slowness that Kornel noticed,
which was caused by our trying to write an entire plaintext
bibliography every time we updated the TOC. We did that because
he had a bibliography inside a branch, and we use plaintext for
creating the tooltip that goes with the branch list.

Other related bugs were fixed along the way. E.g., it turns out
that, if someone had an InsetInclude inside a branch, then we would
have been writing a *plaintext file* for that inset every time we
updated the TOC. I wonder if some of the other reports of slowness
we have received might be due to this kind of issue?

a2e8cb758b Translations for listings insets
The listings inset and include inset of type listings use two english terms
that are not localized yet: "Listing" for the caption and "Listings" for the
list of listings (not supported natively by LyX yet). The existing layout
translation mechanism has been extended to translate these terms as well:

1) Support [[stuff]] context in lib/layouttranslations
2) Support BabelPreamble and LangPreamble in InsetLayout
3) Use a InsetLayout for InsetInclude of type listings
4) Define BabelPreamble and LangPreamble in the layouts for InsetInclude
   and InsetListings

d866717ef7 Integrate texrow with otexstream in order to perform automatic line
counting when exporting to latex. This is done for the code comprised
between \begin{document} and \end{document}, while the preamble code
still needs manual calls to TexRow::newline() for registering new lines.

1ef605f625 Introduce a wrapper class for odocstream to help ensuring that no
blank lines may be inadvertently output. This is achieved by using two
special iomanip-like variables (breakln and safebreakln) in the lyx::
namespace. When they are inserted in the stream, a newline is output
only if not already at the beginning of a line. The difference between
breakln and safebreakln is that, if needed, the former outputs '\n'
and the latter "%\n".
In future, the new class will also be used for counting the number of
newlines issued. Even if the infractrure for doing that is already in
place, the counting is essentially still done the old way.
There are still places in the code where the functionality of the
class could be used, most probably. ATM, it is used for InsetTabular,
InsetListings, InsetFloat, and InsetText.
The Comment and GreyedOut insets required a special treatment and a
new InsetLayout parameter (Display) has been introduced. The default
for Display is "true", meaning that the corresponding latex
environment is of "display" type, i.e., it stands on its own, whereas
"false" means that the contents appear inline with the text. The
latter is the case for both Comment and GreyedOut insets.
Mostly, the only visible effects on latex exports should be the
disappearing of some redundant % chars and the appearing/disappearing
of null {} latex groups after a comment or lyxgreyedout environments
(they are related to the presence or absence of a space immediately
after those environments), as well as the fact that math environments
are now started on their own lines.
As a last thing, only the latex code between \begin{document} and
\end{document} goes through the new class, the preamble being directly
output through odocstream, as usual.

4b1777d2fa Inset::addToToc() can and should be const. It wasn't, I take it, because
of things like:
  pit.push_back(CursorSlice(*this));
which I've had to change to:
  pit.push_back(CursorSlice(const_cast<InsetCaption &>(*this)));
and similarly in a few other places.

If anyone thinks we should instead have:
  explicit CursorSlice(Inset const &);
then we can also do that.

840f2baf91 Simplify some of the bibliography update code. There is really no reason
to be passing the BiblioInfo structure around this way. (That's an old
holdover.) That lets us get rid of the non-const masterBibInfo() again.

e7f2bbe1ee Rename InsetXXX::contextMenu to InsetXXX::contextMenuName. Now this function doesn't need all the parameters and we split the functionality of choosing which context menu to return, from the functionality of supplying the name for the Inset itself. Now, the InsetText context menu is returned for InsetCollapsables when the button is not hit by the mouse. There is no (intended) change in functionality for insets without a button, collapsed insets. This fixes partly bug #6642. A fix for InsetTabular and for insets with no button will follow.

c75d667e78 Organize header files in src\insets.
- Functions that inherit from the same baseclass are grouped together with a command;
- Functions that inherit should not have a doxy entry;
- Other functions are also grouped together as much as possible;
- Some function don't need to be implemented if they return the same as the baseclass;
- and more.

35f3f7cbc4 Don't need this argument, since it was only ever called thus:
it->fillWithBibKeys(d->bibinfo_, it);
This could be useful later, if we decide to try to do the work that
fillWithBibKeys did from updateLabels().

996505c2ae This patch introduces an optional argument to Buffer::updateLabels(), so
as to allow us to call the routine when we are preparing for output and
so to do certain things we might not want to do every time. 

This is an abuse of updateLabels(), in a way, but updateLabels() long
ago became the general recurse-through-the-Buffer routine, and to
implement the sort of thing I want to do here in validate(), say, much
of the code in updateLabels()---in particular, the counter-update
code---would have to be duplicated. So I believe this is the best, and
easiest, way to go.

Actual use of the new argument will follow.

16f119cab5 XHTML output for InsetInclude.
Here's the deal:
* With verbatim, we include it verbatim. This would allow the inclusion of 
  other HTML files.
* With listings, we include it verbatim, wrapped in <pre>.
* With Input and Include, we check if it's a LyX file. If not, we don't do
  anything, since we don't know how to include (say) a TeX file in the HTML
  output. (Wanna call tex4ht, anyone?) If it is a LyX file, we let it write
  itself as HTML, and include it.

39bc3f3f93 Fix most of bug 5539.
If a file fails to load, we do not try to load it again. The downside is that, even if you fixed the problem, then LyX won't try to load it again in that session. I tried resetting the failedtoload_ variable in fileChanged(), but that didn't work, as it doesn't seem to get hit. I'm not sure what to do here, but it isn't a huge issue.
